Respite

Respite is a support that Community Crossroads offers to families that receive Family Support Services.  We provide a monthly financial allocation to the family that is living with the individual with a disability.  This allocation is provided to the family so they can receive a much earned break from care giving.


You can request an application for respite from your support/service coordinator at Community Crossroads or click the link on this page. The application is quite simple and once you have completed and returned the application, we will determine how many hours your family will be eligible for based on the documentation on the application.  If there is not a wait list for respite, you will be notified in writing of your allocation.  As part of that letter, you will receive information regarding monthly submission of your hours as well as the documentation required to receive payment.   The documentation required each month is a simple form called the respite voucher, completed with the name of the person providing respite for your family as well as the hours that they provided the respite.   Annually, you will receive a Respite Agreement that needs to be reviewed, signed and completed in order to continue to receive your respite allocation.  We do retain a list of individuals that have expressed an interest in providing respite.  We can share that list and you are welcome to interview them to determine if you would like to hire them to work with your child.  The person is hired by you, not Community Crossroads.


We receive a specific amount of state and federal funds to provide respite.  We can provide respite when we have available hours.  NH Medicaid allows us the capability to receive the reimbursement for your allocation, if we have available hours.  It is very important that you retain the individuals NH Medicaid for this and many other reasons.  If NH Medicaid is closed, the respite allocation will cease.

Respite Club!

The Respite Club provides respite for families while children with disabilities engage in recreational activities and social interaction. Activities include pizza party, arts & crafts, cooking, music, social groups & large group activities, outdoor fun, smart board, and Wii activities and so much more!
